Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Selma, Alabama. The role supports resident care, evaluation, and treatment planning with an interdisciplinary team.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to establish treatment plans and goals.
• Interpret evaluation findings and communicate recommendations to residents and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
