Job summary

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Document and bill physical therapy services per CMS expectations.
• Evaluate residents and develop short- and long-term treatment goals.
• Perform screenings and determine needs for intervention and treatment.
• Communicate findings and plans to residents, families and interdisciplinary team.
