Job summary

Evolve Therapy Services, LLC, Newport, TN, is seeking a Physical Therapist for an in-house Skilled Nursing Facility. Flexible full-time, part-time, or PRN positions are available to support resident care.

Responsibilities include providing PT services, documenting and billing, evaluating residents, and developing treatment plans with measurable goals while adhering to CMS and department guidelines.

Qualifications

• Must hold a Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of therapy programs.
• Conduct regular resident screenings to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop and communicate short- and long-term treatment goals and plans.
