Job summary

Evolve Therapy Services, LLC seeks a Physical Therapist for an in-house Skilled Nursing Facility in Antioch, TN. Full Time, Part Time, or PRN positions available.

You will plan, implement, and direct physical therapy services following federal, state, and local guidelines to ensure high-quality resident care. The role requires a BS in Physical Therapy, an active license in the state, and ongoing professional development.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services per established standards and procedures.
• Adhere to documentation and billing procedures.
• Evaluate residents and develop treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and the interdisciplinary team.
• Complete CMS-compliant, medically necessary documentation in NetHealth EMR.
