Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Church Hill, TN. This PRN opportunity focuses on delivering high-quality resident care and compliant therapy services.

You will assess residents, develop treatment plans, coordinate with a multidisciplinary team, and ensure CMS and NetHealth documentation is complete and accurate. Active TN PT license and ongoing CEUs are required.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate, plan, implement and follow up resident therapy programs with sound judgment.
• Conduct regular screenings to determine intervention needs and effectiveness of therapy.
• Develop treatment plans with short- and long-term goals; communicate findings to residents and interdisciplinary team.
• Complete CMS/compliant documentation in NetHealth EMR as required.
