Job summary

Evolve Therapy Services, LLC seeks a licensed Physical Therapist for an in-house Skilled Nursing Facility in Antioch, TN. Full Time, Part Time, or PRN positions available.

You will plan, implement, and direct physical therapy services, ensuring CMS-compliant documentation and high-quality resident care. Responsibilities include evaluating residents, developing treatment plans, and communicating progress to families and the interdisciplinary team.

Qualifications

• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and recommendations to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
