Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility in Olivia, MN. The role involves assessing residents, developing and implementing treatment plans, and coordinating with an interdisciplinary team to ensure high-quality care.

Responsibilities include regular screenings, documenting and billing, supervising COTA as needed, and contributing to continuous improvement. Must hold an active OT license and degree in OT.

Qualifications

• Must hold an OT degree and active license to practice in the state(s)
• Ongoing continuing education and professional development to maintain licensure

Responsibilities

• Conduct regular screenings to determine best interventions for residents
• Evaluate residents to develop short- and long-term goals and methods
• Interpret and communicate findings to residents, families, and the team
• Establish resident therapy schedules with the team
• Document and bill Occupational Therapy services per plan
• Oversee COTA as needed and ensure compliance
• Perform other duties as required to treat patients
