Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home therapy, evaluate patients, and contribute to treatment plans. You will assess speech, language, voice and swallowing disorders, administer tests, and document progress in accordance with policy.

The role requires CCC/ASHA credentials, a valid license, CPR, and reliable transportation. Prior home health experience is preferred; you will work under the Clinical Manager with flexible scheduling.

Qualifications

CCC-SLP certification or ASHA CCC required.
Minimum six months experience as a speech-language pathologist.
Current and unrestricted license; CPR certification.
Valid driver's license with transportation to patient sites.

Responsibilities

Evaluate, direct and provide speech/language pathology service in patient homes or facilities.
Participate in Plan of Treatment and Plan of Care development and review.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests and therapeutic applications.
Document changes in patient condition and treatment response for supervision.
Instruct patients in alternative communication systems and provide family coaching.
Prepare written reports and participate in care coordination and discharge planning.
