Job summary

CenterWell Home Health in the United States is seeking a Speech Language Pathologist to provide in-home therapy, evaluating and treating speech, language, voice and swallowing disorders and contributing to the Plan of Treatment and Plan of Care.

You will document progress, coordinate with management and physicians, educate patients and families, and ensure compliance with company policies. Current license and CPR required; home health experience preferred.

Qualifications

• Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license; CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Record and report changes in patient condition to supervisor/physician.
• Instruct patients in alternative communication systems when appropriate.
• Counsel patients, families, and healthcare staff.
• Maintain documentation and submit reports as required.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and professional conduct.

Skills

ASHA CCC or CCC-A

Six months experience as SLP

CPR certification

Valid driver's license
