Job summary

Family First Healthcare Services is seeking a licensed Speech-Language Pathologist (SLP) to provide high-quality home health speech therapy. You will assess communication, cognition, and swallowing, develop treatment plans, and deliver interventions to improve function, safety, and quality of life for patients in Florida.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Florida Speech-Language Pathologist license in good standing.
• ASHA Certificate of Clinical Competence (CCC-SLP) preferred.
• Strong communication, interpersonal, and patient engagement skills.
• Ability to work independently and manage a home health caseload.
• Compassionate, detail-oriented, and committed to high-quality patient care.
• Proficient with computers and electronic medical record documentation.
• Experience with Alora Plus EMR preferred.
• (Preferred) Prior home health experience.

Responsibilities

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home.
• Interpret patient histories, assessments, and physician orders to develop individualized plans of care.
• Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate.
• Educate patients and caregivers on treatment strategies, safety techniques, and carryover exercises.
• Modify treatment plans based on patient progress and evolving clinical needs.
• Collaborate closely with nurses, physicians, case managers, and interdisciplinary team members.
• Maintain accurate, timely, and compliant documentation in the EMR.
• Ensure adherence to federal, state, and agency regulatory standards.
• Stay current with evidence-based practices and professional standards in speech-language pathology.
