Job summary

Elite Home Health is seeking a Speech-Language Pathologist to deliver skilled therapy in patients' homes across Florida. The role focuses on improving communication, cognition, and swallowing while coordinating with an interdisciplinary team.

The SLP conducts evaluations, develops plans of care, and provides education to patients and families, with emphasis on safety and quality outcomes within Medicare guidelines.

Qualifications

• Master's degree in Speech-Language Pathology required, Florida licensure in good standing.
• Minimum of 1 year clinical experience in speech therapy; home health, acute care, or rehab experience preferred.
• Certificate of Clinical Competence (CCC-SLP) preferred, if applicable.
• Current CPR certification required.
• Valid driver's license and reliable transportation; ability to pass background checks.

Responsibilities

• Evaluate speech, language, cognitive, voice, and swallowing disorders in patients' homes.
• Develop and implement individualized speech therapy treatment plans per physician orders and plan of care.
• Provide education to patients and families about goals, strategies, safety, and cognitive support.
• Complete timely clinical documentation, progress notes, and discharge summaries; ensure Medicare/state compliance.
