Job summary

Hometown Home Health in Jacksonville, FL is seeking a licensed Speech-Language Pathologist to provide patient-centered care in patients’ homes. The role requires independence in a home health setting and a passion for improving communication and swallowing abilities.

The position entails evaluations, assessments, and development of individualized treatment plans in coordination with the patient’s physician and care team.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology required or strongly preferred.
• Current Florida SLP license required.
• Current CPR certification; online-only CPR not accepted.
• At least one year of relevant clinical experience.
• ASHA CCC-SLP preferred.
• Strong communication and organizational skills; patient-care focused.

Responsibilities

• Provide speech-language pathology evaluations, assessments, and treatment in patients’ homes.
• Assess patients’ communication, cognitive, and swallowing abilities.
• Develop individualized treatment plans in coordination with the physician and care team.
• Provide therapy to improve communication, speech, language, cognition, and swallowing.
• Educate patients and caregivers on treatment plans and home exercises.
• Monitor progress and adjust treatment plans as needed.
• Complete accurate documentation including visit notes and progress reports.
• Collaborate with physicians, nurses, therapists, and other care team members.
