Job summary

Family First Healthcare Services is seeking a licensed Speech-Language Pathologist (SLP) to join our growing home health team in Florida. You will assess communication, cognitive, and swallowing needs, develop individualized treatment plans, and deliver evidence-based interventions to help patients improve function, safety, and quality of life.

The ideal candidate is patient-centered, organized, clinically strong, and committed to delivering exceptional home-based care.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Active Florida Speech-Language Pathologist license in good standing.
• ASHA CCC-SLP is preferred.
• Strong communication, interpersonal, and patient engagement skills.
• Ability to work independently and manage a home health caseload.
• Proficient with computers and electronic medical record documentation.

Responsibilities

• Conduct comprehensive speech, language, cognitive, and swallowing evaluations in the home.
• Interpret patient histories, assessments, and physician orders to develop individualized plans of care.
• Provide therapeutic interventions for speech, language, voice, cognition, and dysphagia as appropriate.
• Educate patients and caregivers on treatment strategies, safety techniques, and carryover exercises.
• Modify treatment plans based on patient progress and evolving clinical needs.
• Collaborate closely with nurses, physicians, case managers, and interdisciplinary team members.
• Maintain accurate, timely, and compliant documentation in the EMR.
• Ensure adherence to federal, state, and agency regulatory standards.
• Stay current with evidence-based practices and professional standards in speech-language pathology.
