Job summary

CenterWell Home Health, a Humana company, seeks a Home Health Speech Language Pathologist to evaluate and treat disorders of speech, voice, language, hearing and swallowing in patients at home or in facilities. You will develop treatment plans, coordinate care, and document progress to support rehabilitation and independence.

Required are ASHA CCC-SLP certification, a current license, CPR, six months of experience as an SLP, and reliable transportation. Home Health experience is preferred.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in home or facility
• Participate in development and periodic review of Plan of Treatment and Plan of Care
• Assess and treat disorders of speech, voice, language, hearing and swallowing to improve function
• Administer and interpret diagnostic tests and applications of therapeutic treatments
• Observe and report changes in patient condition to supervisor/physician
• Provide instruction on alternative communication systems when appropriate
• Counsel patients, families and healthcare staff
• Maintain comprehensive documentation and reports as required by policy
• Participate in care coordination and discharge planning
• Maintain confidentiality and adhere to privacy standards
