Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and provide SLP services in patients' homes or facilities. You will participate in treatment planning, assess speech, voice, language, hearing and swallowing disorders, and document progress and care plans.

Strong communication with families and staff is essential. The role requires ASHA CCC-SLP/CCC-A certification, six months of relevant experience, a current license, and CPR certification.

Qualifications

• ASHA certified clinician required.
• Six months of speech-language pathology experience minimum.
• Current license to practice speech-language pathology.
• CPR certification current.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments.
• Observe and report changes in patient condition to supervisor/physician.
• Instruct patients on use of alternative communication systems when appropriate.
• Provide counselling to patients, families and healthcare staff.
• Submit written reports of evaluations, visits, care plans and progress.
• Participate in care coordination and discharge planning.
