Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to evaluate and treat patients in home or facility settings, focusing on rehabilitation and functional communication. You will develop and review plans of treatment, document progress, and collaborate with the care team.

Requirements include ASHA CCC (or Audiology), at least six months of experience, active license, CPR, and a valid driver’s license with reliable transportation.

Qualifications

• ASHA CCC (Speech-Language Pathology) or Audiology certification is required.
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted professional license.
• CPR certification is required.
• Valid driver’s license, automobile insurance and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in home or facility.
• Participate in development and periodic review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize impairment.
• Administer and interpret diagnostic tests and therapeutic applications including audiology screening.
• Observe and report changes in patient condition to supervisor/physician.
• Provide training to patients in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Prepare and submit written evaluations, visits, care plans, and progress reports.
