Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to evaluate and provide rehabilitation services to patients in home or facility settings.

You will develop and review Plans of Treatment, treat speech, voice, language and swallowing disorders, and coordinate with the clinical team to support patient independence and recovery.

This patient-facing role requires ASHA CCC, a valid license, CPR certification and a driver's license to travel to patient homes as needed.

Qualifications

• ASHA CCC-SLP certification or equivalent required.
• Current CPR certification required.
• Valid driver's license required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in home or facility.
• Participate in development and periodic review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction to patients and families in communication strategies.
