Job summary

Guardian Angel Home Care Inc. is seeking compassionate Speech-Language Pathologists (SLPs) to join our growing nationwide team. We offer part-time and PRN opportunities to help patients improve communication, cognitive function, swallowing abilities, and quality of life in the comfort of their homes.

You will collaborate with physicians, nurses, therapists, patients, and caregivers to implement individualized treatment plans that promote independence and successful outcomes.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license in the state of practice.
• CCC-SLP certification preferred.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
