Job summary

Guardian Angel Home Care is seeking compassionate Speech-Language Pathologists (SLPs) to join our growing nationwide team for home-based care. You will evaluate and treat speech, language, cognitive, and swallowing disorders, and develop individualized plans to improve communication and quality of life in patient homes.

You will collaborate with physicians, nurses, therapists, patients, and caregivers, educate families, and document progress while complying with all regulations.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license in the state of practice.
• CCC-SLP (preferred).
• Current CPR certification (or willingness to obtain).
• Valid driver's license, reliable transportation, and automobile insurance.
• Strong communication, organizational, and time management skills.
• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
