Guardian Angel Home Care Inc. is seeking compassionate Speech-Language Pathologists (SLPs) to join our nationwide team, delivering care in patients' homes.

You will evaluate speech, language, cognitive, and swallowing disorders and develop individualized treatment plans in collaboration with physicians, nurses, and other clinicians. You will provide skilled therapy, educate patients and families, complete documentation, and participate in care planning.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current SLP license in the state of practice.
• CCC-SLP certificate preferred.
• Current CPR certification or willingness to obtain.
• Valid driver's license and reliable transportation.
• Strong communication, organizational, and time management skills.

Responsabilités

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
