Guardian Angel Home Care Inc. is seeking compassionate Speech-Language Pathologists (SLPs) to join our nationwide team on a part-time and PRN basis.

Clinicians will evaluate, treat, and educate patients in their homes to improve communication, cognition, and swallowing, while collaborating with an interdisciplinary team. Responsibilities include developing individualized plans, documenting progress, and ensuring regulatory compliance.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current SLP license in the practicing state.
• CCC-SLP certification preferred.
• CPR certification current or willingness to obtain.

Responsabilités

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
