Job summary

Guardian Angel Home Care is seeking compassionate Speech-Language Pathologists (SLPs) to join our nationwide team for Home Health services. Part-Time and PRN roles available, with opportunities to collaborate with physicians, nurses, therapists, patients and caregivers in the home setting.

The role involves evaluating and treating speech, language, cognitive, voice and swallowing disorders, developing individualized plans, and maintaining thorough documentation to ensure high-quality patient

Qualifications

• Master's Degree in Speech-Language Pathology from an accredited program.
• Current SLP license in the practicing state.
• CCC-SLP certification preferred.
• CPR certification or willingness to obtain.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services adhering to professional standards and policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers on treatment plans and home exercises.
• Collaborate with interdisciplinary team to ensure coordinated care.
• Complete timely clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor progress and adjust treatment plans accordingly.
• Maintain compliance with all regulations.
