Job summary

Guardian Angel Home Care is seeking compassionate Speech-Language Pathologists to join our nationwide team for part-time and PRN opportunities. Therapists will help patients improve communication, cognitive function, and swallowing while delivering care in the home setting.

You will work with physicians, nurses, therapists, patients, and caregivers to develop and implement individualized treatment plans, promoting independence and positive outcomes.

Qualifications

• Master's Degree in Speech-Language Pathology from an accredited program.
• Current SLP license in the state of practice.
• CCC-SLP certification preferred.
• CPR certification current or willingness to obtain.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
