Job summary

Guardian Angel Home Care is seeking compassionate Speech-Language Pathologists to join our growing nationwide team, providing home-based therapy.

You will evaluate, treat, and educate patients with speech, language, cognitive, and swallowing disorders, collaborating with physicians and caregivers to achieve meaningful outcomes.

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license in the state of practice.
• Certificate of Clinical Competence (CCC-SLP) preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and automobile insurance.
• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
