Guardian Angel Home Care is seeking compassionate Speech-Language Pathologists (SLPs) for part-time and PRN roles nationwide. You will evaluate speech, language, cognitive, voice, and swallowing disorders and create individualized treatment plans to help patients improve communication and quality of life in the home setting.

You will collaborate with physicians, nurses, therapists, patients, and caregivers, document care, and ensure compliance with regulatory standards while supporting patient

Competenze

• Master's Degree in Speech-Language Pathology from an accredited program.
• Current Speech-Language Pathologist license in the state of practice.
• Certificate of Clinical Competence (CCC-SLP) preferred.
• Current CPR certification (or willingness to obtain).
• Valid driver's license, reliable transportation, and automobile insurance.

Mansioni

• Evaluate patients with speech, language, cognitive, voice, and swallowing disorders.
• Develop individualized treatment plans based on physician orders and patient needs.
• Provide skilled speech therapy services in accordance with professional standards and agency policies.
• Assess and treat communication, cognitive-linguistic, and swallowing impairments.
• Educate patients, families, and caregivers regarding treatment plans and home exercise programs.
• Collaborate with interdisciplinary team members to ensure coordinated patient care.
• Complete timely and accurate clinical documentation.
• Participate in case conferences and care planning meetings as needed.
• Monitor patient progress and adjust treatment plans accordingly.
• Maintain compliance with all federal, state, and agency regulations.
