Job summary

LiveWell Homecare Agency is seeking a Home Health Speech Language Pathologist to deliver personalized speech, voice, language, hearing, and swallowing therapy in patients’ homes or facilities.

Responsibilities include evaluating patients, developing treatment plans, administering diagnostic tests, and implementing interventions, while observing and reporting progress. Strong CPR certification, license, and reliable transportation are required; home health experience is preferred.

Qualifications

• Six months of experience in speech-language therapy or a related role.
• Unrestricted license to practice speech-language pathology.
• CPR certification.
• Reliable transportation with a valid driver’s license.
• Preference for candidates with home health experience.

Responsibilities

• Evaluate patients in their homes or facilities and develop individualized treatment plans.
• Administer diagnostic tests as part of assessment.
• Implement and monitor therapeutic interventions and progress.
• Observe and report patient progress to care teams and families.
• Instruct patients and families on communication aids and strategies.
• Provide counseling and support to patients and caregivers.
• Maintain thorough documentation of treatment sessions and outcomes.
• Participate in care coordination and discharge planning.
