Job summary

Palms Home Care in Tampa, FL is seeking a Speech Language Pathologist to provide in-home therapy services. You will work independently while coordinating care with an interdisciplinary team and document visits per Medicare guidelines.

Responsibilities include comprehensive evaluations, OASIS knowledge, treatment of speech, language, cognitive, and dysphagia disorders, plus education for patients and caregivers. The position offers a flexible schedule and mileage reimbursement.

Qualifications

• Master’s degree in Speech Language Pathology.
• Active Florida SLP license.
• Home health experience preferred.
• Knowledge of OASIS documentation is a plus.
• Possesses and maintains current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments
• Develop and implement individualized care plans
• Treat patients with speech, language, cognitive, and dysphagia disorders
• Accurately document visits in accordance with Medicare guidelines
• Educate patients and caregivers on exercises and compensatory strategies
• Communicate effectively with physicians and care team members
