First Senior Home Health Tampa seeks a licensed speech-language pathologist to provide in-home care on an intermittent basis, following physician orders, and to develop and revise treatment plans.

You will assess patient needs, conduct OASIS evaluations, coordinate with the care team, and educate patients and families about the Speech Therapy program.

The role requires strong communication, the ability to work independently in patients' homes, and adherence to clinical guidelines.

Qualifications

• Must have master's or doctoral degree in speech-language pathology and licensure in the practicing state.
• Licensed or registered as an SLP in Florida.
• Preferred ASHA membership.
• Must have at least one year of institutional experience.
• Criminal background check required.
• Current CPR certification; online certification not accepted.

Responsabilités

• Provide skilled speech pathology services in patients' homes according to physician orders.
• Evaluate functioning and recommend techniques to enhance hearing.
• Develop and adjust speech therapy plans with physician approval.
• Record evaluation data and progress notes within 48 hours.
• Coordinate with interdisciplinary team and educate patients and families.
• Arrange outpatient services when needed.
