Job summary

Palms-Home-Care in Florida is expanding and seeks a dedicated Speech Language Pathologist to provide in-home therapy services to patients.

The ideal candidate will work independently while coordinating care with an interdisciplinary team, have home health experience, and hold an active Florida SLP license; knowledge of OASIS documentation and CPR certification are a plus, and a valid driver’s license with reliable transportation is required.

Qualifications

• Master’s degree in Speech Language Pathology.
• Active Florida SLP license.
• Home health experience preferred.
• Knowledge of OASIS documentation is a plus.
• Possesses and maintains current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments
• Develop and implement individualized care plans
• Treat patients with speech, language, cognitive, and dysphagia disorders
• Accurately document visits in accordance with Medicare guidelines
• Educate patients and caregivers on exercises and compensatory strategies
• Communicate effectively with physicians and care team members
