Job summary

Diamond Care Group is seeking a dedicated Speech Language Pathologist to provide in-home therapy services across Florida. Ideal candidates will have home health experience, work independently, and coordinate care with an interdisciplinary team to deliver high-quality patient outcomes.

Responsibilities include evaluations, care plan development, treatment for speech and cognitive disorders, and Medicare-compliant documentation.

Qualifications

• Master’s degree required in Speech Language Pathology
• Active Florida SLP license required
• CPR certification current
• Home health experience preferred
• Valid driver’s license and reliable transportation

Responsibilities

• Perform comprehensive evaluations and OASIS assessments
• Develop and implement individualized care plans
• Treat patients with speech, language, cognitive, and dysphagia disorders
• Accurately document visits according to Medicare guidelines
• Educate patients and caregivers on exercises and compensatory strategies
• Communicate effectively with physicians and care team members
