Job summary

CenterWell Home Health, a Humana company, seeks a Speech Language Pathologist to deliver in-home care, reporting to the Clinical Manager. You will evaluate and treat patients at home or in facilities, focusing on rehabilitation and functional improvement to help patients regain independence.

You will develop treatment plans, perform diagnostic tests, and train patients in alternative communication. A valid ASHA CCC, licensure, CPR, and a driver's license are required along with six months'

Qualifications

Meet the education and experience requirements for CCC in SLP or Audiology per ASHA.
Minimum six months' experience as a speech language pathologist.
Current and unrestricted license to practice.
CPR certification is required.
Valid driver's license, auto insurance and reliable transportation.

Responsibilities

Evaluate, direct and provide speech-language pathology services to patients in home or facility.
Participate in development and review of Plan of Treatment and Plan of Care.
Assess and treat disorders of speech, voice, language, hearing and swallowing.
Administer and interpret diagnostic tests including audiologic screening.
Observe, record and report changes in patient condition to supervisor/physician.
Provide instruction for use of alternative communication systems when appropriate.
Counsel and instruct patients, families and healthcare staff.
Prepare written reports of evaluations, visits, care plans and progress notes.
Participate in care coordination and discharge planning.
Maintain confidentiality and follow company policies on information sharing.
