Job summary

CenterWell Home Health seeks a Speech Language Pathologist to deliver in-home or facility-based therapy, evaluating and treating disorders of speech, language, voice, and swallowing and guiding plans of care.

Requirements include ASHA CCC, a current unrestricted license, CPR, and a valid driver's license. Six months of home health experience is a plus; strong organization and communication skills are essential.

Qualifications

• Meets the education and experience requirements for ASHA CCC in Speech-Language Pathology or Audiology.
• Minimum six months experience as a speech therapist or SLP.
• Current and unrestricted professional license.
• CPR certification and valid driver’s license.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in patients' homes or facilities.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments including audio logic screening.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain required documentation.
• Prepare timely written reports of evaluations, visits, care plans and progress.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and adhere to company policy; attend staff development and CEU activities.
