Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to evaluate and treat patients in their homes or facilities, supporting rehabilitation and functional communication skills. You will participate in care planning, document progress, and coordinate with physicians and families to maximize independence.

Requirements include ASHA CCC, at least six months of SLP experience, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• CCC in Speech-Language Pathology per ASHA.
• Six months of SLP experience required.
• Home health experience is a plus.
• Current CPR certification required.
• Valid driver's license, auto insurance, and reliable transportation.
• Strong organizational and communication skills.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Participate in Plan of Treatment and Plan of Care development/review.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments including aural/lingual screening.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain necessary documentation.
