Job summary

CenterWell Home Health seeks a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You will develop plans of care, assess speech, voice, language, hearing and swallowing disorders, and provide patient and family instruction while maintaining documentation.

The role requires ASHA CCC or equivalent certification, a current license, CPR, and reliable transportation for home visits. TB and driver-safety programs apply.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments.
• Observe, record and report changes in the patient's condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit documentation as required by the Company policy.
• Prepare progress reports and care plans; participate in care coordination and discharge planning.
