Job summary

CenterWell Home Health seeks a Home Health Speech Language Pathologist to evaluate and treat patients in home or facility settings. You’ll participate in care planning, document progress, and coordinate with the care team to improve communication and swallowing disorders.

Responsibilities include administering diagnostic tests, training patients in alternative communication, and ensuring confidentiality of patient information. A valid license and CPR certification are required.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Observe, report changes in patient condition to supervisor/physician.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation and progress reports as required by policy.
• Participate in care coordination and discharge planning.
