Job summary

CenterWell Home Health is seeking a Home Health Speech-Language Pathologist to evaluate and provide speech-language pathology services in patients' homes or facilities. You will participate in plans of treatment and care, assess speech, language, voice and swallowing disorders, and document progress.

Qualifications

• Minimum six months experience as a speech therapist/speech-language pathologist.
• Current CPR certification required.
• A valid driver’s license and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
• Participate in development/review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; provide therapy and documentation.
