Job summary

CenterWell Home Health is seeking a Home Health Speech-Language Pathologist to deliver in-home or facility-based services. You will evaluate and treat speech, voice, language and swallowing disorders, develop treatment plans, and coordinate care with a multidisciplinary team to improve patient independence and quality of life.

The role requires ASHA CCC- related certification or eligibility, current CPR, and a valid driver’s license with reliable transportation, along with a minimum six months

Qualifications

• Must meet the education and experience requirements for Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in the home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments including audio logic screening.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction in use of alternative communication systems when appropriate.
