Job summary

CenterWell Home Health is seeking a licensed speech-language pathologist to provide in-home therapy, focusing on rehabilitation and functional improvement for patients in their homes or facilities. You will evaluate, treat, and monitor communication and swallowing disorders, collaborate with a clinical manager, and document progress while maintaining high professional standards and safety protocols.

The role requires current licensure, CPR, a valid driver's license, and reliable transportation;

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat speech, voice, language, hearing and swallowing disorders to improve outcomes.
• Administer and interpret diagnostic tests and applications of therapy.
• Observe and report changes in the patient’s condition to supervisor/physician.
• Provide instruction for alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation and reports.
• Participate in care coordination and discharge planning.
