Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes, advancing rehabilitation and communication.

You will report to the Clinical Manager, develop Plans of Treatment, coordinate with care teams, and educate patients and families.

Requirements include ASHA CCC-SLP or CCC-A, a valid state license, CPR, a valid driver’s license with reliable transportation, and at least six months of experience.

Qualifications

• ASHA CCC-SLP or CCC-A certification required.
• Minimum six months experience as a speech therapist / speech-language pathologist.
• Current and unrestricted state license.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation required.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in the home or facility.
• Develop and periodically review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic treatments including audio logic screening.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Counsel and instruct patients, families and healthcare staff.
• Maintain documentation and submit reports of evaluations, visits, care plans and progress.
• Participate in care coordination and discharge planning.
• Maintain confidentiality and comply with company policies.
