Job summary

CenterWell Home Health seeks a Speech Language Pathologist to deliver in-home evaluations and treatments, collaborating with a clinical team to improve patient outcomes. You will participate in care plans and coordinate services for patients in home settings.

The role requires ASHA CCC-SLP, active state license, CPR, and reliable transportation. Home health experience is preferred. This is a full-time position with competitive compensation.

Qualifications

• Must hold Certification of Clinical Competence in Speech-Language Pathology (ASHA).
• Minimum six months experience as a speech therapist/speech-language pathologist.
• Current and unrestricted state license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to minimize effects.
• Administer and interpret diagnostic tests and therapeutic treatments including audiologic screening.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation and reports as required.
• Participate in care coordination and discharge planning.
