Job summary

CenterWell Home Health seeks a qualified Speech Language Pathologist to evaluate and direct speech-language pathology services in patients' homes or facilities. You will participate in plans of treatment and care, assess and treat speech, voice, language, hearing and swallowing disorders, and document progress and results.

The role requires ASHA certification, a current license, CPR, and transportation. Experience in home health is preferred, with established patient care coordination and safety

Qualifications

• ASHA CCC or ASHA certification as required.
• Six months of experience as speech-language pathologist.
• Current and unrestricted license; CPR certification.

Responsibilities

• Evaluate, direct and provide speech/language pathology services to patients in home or facility.
• Develop and review Plan of Treatment/Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests.
