Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home therapy, evaluate and treat patients, and participate in plans of care. You will work under the Clinical Manager to help patients regain speech, swallowing and communication skills in their own homes.

The role requires at least six months of experience, a valid license, CPR, and reliable transportation. Per-visit pay ranges from $54 to $76, with an annual salary band available for full-time hours.

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.
• Home Health experience (preferred).

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests including audiologic screening.
• Observe and report changes in patient condition to supervisor/physician.
• Provide instruction and training on alternative communication systems.
• Maintain documentation and submit progress reports as required.
• Participate in care coordination and discharge planning.
