Job summary

CenterWell Home Health is seeking a Home Health Speech Language Pathologist to evaluate and treat patients in their homes or facilities under the Clinical Manager. You will develop and review treatment plans, document progress, and collaborate with the care team to improve swallowing, communication and overall quality of life.

Required are at least six months of SLP experience, an unencumbered license, CPR certification, and a valid driver's license with insurance.

Qualifications

• Six months minimum experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services to patients in home or facility settings.
• Participate in development and review of Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Maintain and submit documentation and reports as required by policy.
