Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to provide in-home services under the Clinical Manager. You will evaluate, diagnose and treat communication and swallowing disorders, develop plans of care, and collaborate with patients and families to improve function and quality of life.

This role requires ASHA CCC, a minimum of six months experience, an unrestricted license, CPR, and a valid driver's license with reliable transportation.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
• Minimum six months experience as a speech therapist / speech language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify, evaluate and minimize effects of such disorders
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening
• Observe, record and report changes in the patient's condition and response to treatment to supervisor and/or physician
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation as required by the Company and/or facility; prepare and submit reports
• Participate in care coordination and discharge planning
• Maintain confidentiality and share information only as required by policy
