Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to evaluate and treat patients in home or facility, develop Plans of Treatment, and use professional judgment to address speech, voice, language, hearing and swallowing issues.

Requirements include ASHA CCC, active licensure, CPR, and at least six months of home health experience. Valid driver’s license and strong organizational skills are needed. Compensation includes pay per visit and a wide annual range.

Qualifications

• Must meet ASHA CCC education and experience requirements.
• Active state license as Speech-Language Pathologist or Audiologist.
• CPR certification and current certification status.
• Minimum six months of speech therapy/home health experience.

Responsibilities

• Evaluate, direct and provide speech-language pathology services in home or facility.
• Develop and periodically review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and therapeutic applications.
• Record changes and report to supervisor/physician; discharge planning.
• Provide instruction on alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff; maintain documentation.
• Submit reports and participate in care coordination activities.
