Qualifications

• Ability to work independently with a flexible schedule.
• Must have a personal vehicle and valid driver’s license; mileage reimbursed.

Responsibilities

• Uses person-centered planning and service delivery methods in developing and implementing plans.
• Facilitates regular communication with the person and IDT regarding supports and services’ effectiveness.
• Provides written documentation on daily progress related to plans and related paperwork.
• Produces reports regarding accidents or incidents and follows notification guidelines.
• Facilitates intake and IDT meetings.
• Maintains data confidentiality and case files.

Education

Baccalaureate degree in human services

Associate degree in human services

Diploma in human services

50 hours of education and training related to human services

Four years of direct care experience under supervision
