Supportive Concepts for Families, Inc. seeks a Community Program Coordinator to lead CPS and In-Home and Community Support services for individuals with IDD, DD, and ASD.

You will ensure compliant delivery of person-centered services, promoting independence, skill development, and community inclusion. You will collaborate with leadership to ensure high-quality supports, manage documentation, and align with funding requirements, while mentoring staff and expanding service offerings to meet client

Competenze

• Bachelor’s degree in Human Services, Social Work, Psychology, Rehabilitation, or related field preferred.
• Experience working with individuals with IDD, DD, and/or ASD.
• Knowledge of CPS, IHCS, ISPs, documentation standards, and billing requirements.
• Must become ACRE Certified or CESP Certified within 6 months of hiring.
