Job summary

Lawndale is hiring a PACE Home Care Coordinator in Chicago, IL to support the occupational therapist and Center Manager in developing and implementing home care services with patient wellbeing as the focus. The role requires at least one year of home or community health experience and involves coordinating plans with providers, training aides, and ensuring compliance.

Responsibilities include initial and 6-month assessments, IDT participation, and EVV/documentation oversight.

Qualifications

• Must be able to work in a patient home environment.
• Must possess organizational and problem-solving skills.
• Must be able to work effectively in a team environment.
• Must be able to treat patients of all ages including older adults.
• Must be able to move, lift and assist patients with dexterity and communication.
• Must demonstrate compassionate and responsive care per behavioral standards.
• Willing to use personal vehicle with mileage reimbursement.
• At least one year of experience in home or community health.
• Supervisory experience preferred.
• Licensure preferred but not required as COTA/LPN/CNA in IL.

Responsibilities

• Collaborate with the occupational therapist to assess home care needs.
• Coordinate home care plans with LCHC and contracted providers.
• Train home care aides on tasks, expectations, and the PACE model of care.
• Perform initial assessments and 6-month reassessments, including in-home visits.
• Participate in daily IDT meetings and provide updates on home care plans.
• Maintain cross-training to ensure continuity of services.
