Job summary

The PACE Place seeks a Home Care Coordinator to develop and supervise in-home care plans for PACE Partners participants in Jacksonville, FL. You will coordinate nursing, therapy, and personal care services, ensuring plans align with IDT decisions and patient needs.

Ideal candidates have at least 1 year of eldercare/home health experience, current LPN/RN licensure, and strong organizational skills for comprehensive care management.

Qualifications

• 1+ year experience with frail or elderly patients, preferably in home health or long-term care.
• Current LPN or RN license; valid driver's license and auto insurance.
• Strong organization, documentation, and communication skills.

Responsibilities

• Assess homecare needs and develop plans of care with IDT.
• Coordinate home health services (nursing, PT, OT, SLP, personal care).
• Conduct routine home visits and adjust services as needed.
• Oversee scheduling of medical escorts and ensure quality care.
• Maintain compliant documentation and participate in IDT meetings.
