Description

Position Summary
The PACE Home Care Coordinator supports the PACE Occupational Therapist and Center Manager by ensuring the development and implementation of homecare services in accordance with established procedures with participant well-being as the central focus.

Principal Duties and Responsibilities*
Participant Assessment & Care
  • Collaborate with the occupational therapist to assess and understand the home care needs of a frail elderly population, including ADL and IADL.
  • Coordinate implementation of all home care plans of care through both LCHC and contracted providers to ensure quality and alignment with participant needs.
  • Train and communicate clearly with assigned home care aides the tasks, expectations, PACE model of care, and compliance requirements.
  • Complete required initial assessment and 6-month reassessments, including in-home evaluations.
  • Actively participate in daily Interdisciplinary Team (IDT) meetings and provide updates on home care plans.
  • Maintain cross-training to provide backup support and ensure continuity of services.
Compliance
  • Monitor documentation from both internal and contracted home care staff to ensure alignment with the plan of care.
  • Coordinate and oversee home care scheduling, including all schedule changes for internal and external aides.
  • Process supply requests from home care staff and coordinate participant communication for purchases when required.
  • Ensure compliance with home care tasks completed in the day center setting (e.g., showers, laundry) and communicate expectations to the Center Manager and team.
  • Conduct periodic home audits to ensure compliance and quality of care.
  • Periodically review Electronic Visit Verification (EVV) records to ensure services align with the plan of care.
  • Perform other tasks as assigned.
Requirements
  • Must be able to work in a patient home environment.
  • Must possess organizational and problem-solving skills.
  • Must be able to work effectively in a team environment.
  • Must be able to treat patients of all ages including older adults
  • Must be able to move about freely, lift and turn patients, possess manual dexterity and visual acuity to see and treat patients, be able to hear and orally communicate.
  • Must demonstrate the ability to deliver such services to the clinical team, patients and fellow employees with compassion and in a responsive, courteous and concerned manner in accordance with the Standards of Behavior.
  • Must be willing to use their personal vehicle (mileage can be reimbursed).
  • At least one year experience in home or community health or similar home-based services for older adults.
  • Supervisory experience preferred.
  • Licensure preferred but not required as a COTA or LPN or CNA in the State of Illinois
